As per required by the law (42 CFR 422.503 & 42 CFR 423.504) and Jade Health Care Medical Group, you and your staff must complete your Annual Fraud, Waste and Abuse (FWA) training, as well as your Basic Compliance training. If you have already done one with another medical group, please email in your training certificate.

FWA and Basic Compliance Training unit must be completed before the end of the year. The Attestation forms must be completed and sent in before the end of the year.

Please read the Jade Compliance Training Presentation below and fill out the Jade Provider Attestation form provided below. If you are a provider under San Francisco Health Plan (SFHP), please download the appropriate SFHP documents below and fill out the SFHP Attestation Form. Once completed, please send the completed form(s) to one of the two options below:
